1. About Re:Formd

These Terms are an agreement between you and Retrospect LLC, the owner and operator of Re:Formd ("Re:Formd", "we", "us"). They apply to every way you use Re:Formd, including the mobile app and the website.

Re:Formd is an AI-powered health and wellness advisory platform. It helps you understand your health information and make informed decisions about your well-being. Re:Formd is not a healthcare provider and does not diagnose, prescribe or provide medical treatment.

2. Eligibility and your account

  • You must be at least 18 years old to use Re:Formd.
  • One account per person. Give accurate information, and keep your sign-in details secure. You are responsible for activity on your account.
  • Tell us promptly at support@getreformd.com if you think your account has been accessed without your permission.

3. Sabrina, your AI Health Advisor

Sabrina is Re:Formd's AI Health Advisor. She uses artificial intelligence and the information available to the platform, including what you choose to share, to provide personalized educational information, analysis, insights and wellness recommendations.

  • Sabrina is artificial intelligence, not a licensed healthcare professional.
  • AI can make mistakes. Responses may be inaccurate, incomplete, outdated or wrong for your circumstances, including when they sound confident.
  • Sabrina does not diagnose conditions, prescribe medications or provide medical treatment, and she doesn't verify that the data you enter or upload is accurate.
  • You remain responsible for your healthcare decisions. Consult a qualified professional before making significant decisions about medications, treatments or medical conditions.
  • Sabrina is designed not to advise you to start, stop or change the dose of a medication. Like any AI she can still make mistakes, so always confirm medication decisions with your prescriber.
  • These points are part of these Terms. Accepting these Terms is how you acknowledge them. There is no separate AI acknowledgment.

4. Educational use and medical limitations

Content in Re:Formd, including Health State Reports, scores, Re:Formd Age and recommendations, is educational and informational. It is not medical advice, a diagnosis, a treatment plan or a prescription. Using Re:Formd does not create a doctor–patient, therapist–patient or other healthcare-provider relationship. Don't delay or disregard professional medical advice because of something in Re:Formd. You are responsible for the accuracy and completeness of the health information you provide.

5. Not an emergency service

Sabrina is an automated AI system. Conversations are not continuously monitored by medical professionals or emergency response personnel. Re:Formd is not an emergency response service. If you believe you are experiencing an emergency, contact emergency services immediately (in the US, call 911; for a mental-health crisis, call or text 988).

6. Membership, subscriptions and payments

  • Free and Membership. Some features are available without charge. Re:Formd Membership is a paid subscription. What is included is described in the app; resource limits apply to AI features so the service stays available to everyone.
  • Price and period. The price, billing period and any free trial or introductory offer are shown before you subscribe.
  • Automatic renewal. Your Membership renews automatically at the end of each billing period at the then-current price until you cancel. For a free trial or introductory offer, billing at the regular price starts when it ends unless you cancel before then.
  • Purchases through Apple. Membership is sold as an in-app purchase through the App Store. If you subscribe in the iOS app, Apple processes the payment and your purchase is also governed by Apple's terms, including Apple's standard Licensed Application End User License Agreement. Apple manages billing, renewals and refunds for those purchases.
  • Price changes. We give advance notice of price changes as the store or applicable law requires.
  • Complimentary access. We may grant complimentary Membership for a stated period. It ends on the stated date unless you subscribe.

7. Cancellation and refunds

  • Apple subscriptions: cancel any time in your Apple ID subscription settings (Account → Membership in the app links there). Deleting the app or your Re:Formd account does not cancel an Apple subscription. Refund requests for App Store purchases go to Apple, which decides them.
  • After cancellation, Membership continues until the end of the period you paid for, then your account returns to the free plan. Your health record stays available under our Privacy Policy.

8. Substances and high-risk content

Re:Formd provides educational information about supplements, medications, peptides, SARMs, research compounds and recreational substances. People make personal health decisions regardless, and we believe clear, responsible information is safer than experimentation without context. Re:Formd does not encourage, recommend or promote the use of any substance. Re:Formd does not offer a peptide dosing or reconstitution calculator.

9. Acceptable use

Don't:

  • use Re:Formd unlawfully, or to harm anyone;
  • access another person's account or data without permission;
  • probe, scrape, overload or circumvent limits or security controls, including AI usage limits;
  • reverse engineer the service, or use it to build a competing product;
  • upload content you don't have the right to share, or content that is unlawful;
  • misrepresent Re:Formd's outputs as professional medical advice to others.

10. Your content

You keep ownership of the content and data you provide. You give Retrospect LLC a limited licence to host, process, display and transmit it only to operate and improve Re:Formd for you, as described in the Privacy Policy. We do not sell your personal or health information.

11. Third-party services

Re:Formd works with services you choose to connect (for example WHOOP) and with providers that help us run it. Your use of a connected service is also governed by that service's terms. We are not responsible for third-party services, but we choose our providers carefully. The Privacy Policy lists them.

12. Coaches and organizations

If you connect a coach, gym or organization, you choose what they can see. Your coach always sees the plans they assign you and your progress on them. Managers at the organization see whether you're on track with the plans your coach assigns (status and completion counts), never your record. They are independent of Re:Formd unless we say otherwise, and their advice is their own responsibility. You can end a connection at any time.

13. Intellectual property

Re:Formd, including its name, trademarks, logos, software, algorithms, models, design, documentation and content, belongs to Retrospect LLC or its licensors and is protected by law. Don't copy, distribute, reverse engineer or commercially exploit any part of it without written permission.

14. Disclaimer of warranties

To the extent permitted by law, Re:Formd is provided "as is" and "as available", without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose, accuracy and non-infringement. We do not guarantee health outcomes, medical improvements, disease prevention or performance results. You use Re:Formd at your own risk. Some jurisdictions don't allow certain exclusions, so some of these may not apply to you.

15. Limitation of liability

To the maximum extent permitted by law, Retrospect LLC is not liable for indirect, incidental, special, consequential or punitive damages, or for health outcomes or personal decisions made using Re:Formd. Nothing in these Terms limits liability that cannot be limited by law.

16. Termination and account deletion

  • You may stop using Re:Formd at any time and delete your account in the app (Account → Privacy & Data → Delete account), or request deletion at getreformd.com/data-controls. What is deleted and what is retained, and for how long, is described in the Privacy Policy.
  • We may suspend or terminate accounts that violate these Terms, misuse or attempt to exploit Re:Formd, or engage in illegal activity.
  • Deleting your account does not cancel an Apple subscription (see §7).
  • Sections that by their nature should survive termination (for example 10, 13, 14, 15 and 17) survive.

17. Governing law

These Terms are governed by the laws of the State of Arizona, United States, without regard to conflict-of-law rules.

18. Changes to these Terms

We may update these Terms. For material changes we will notify you in the app or by email before they take effect, and where required we will ask you to accept the updated Terms before continuing to use Re:Formd. The version and date are shown at the top.
