1. Welcome to Re:Formd

These Terms form a single agreement between you and Retrospect LLC, the owner and operator of Re:Formd. In these Terms, “Re:Formd,” “we,” “our,” and “us” refer to Retrospect LLC unless the context requires otherwise. These Terms apply to all access methods (for example, the mobile application and the website).

Re:Formd is a health education organization and personal insight platform. It is not a medical provider, diagnostic system, or treatment service.

Re:Formd helps people better understand their bodies, health signals, and the systems that influence wellness. We provide tools, educational information, and structured insights designed to help you organize and better understand your health.

Content in Re:Formd is provided for educational and informational purposes only.

Nothing in Re:Formd constitutes:

What Re:Formd is not

• Not for emergencies. Do not use Re:Formd for medical emergencies. In an emergency, contact emergency services or your doctor.

Your responsibility for health data

Users may enter personal health information including symptoms, conditions, medications, supplements, lab data, and lifestyle factors. Users remain responsible for the accuracy and completeness of their data. We do not verify your health data for clinical accuracy. 8. AI transparency and assisted features

Re:Formd includes AI-assisted capabilities, including in-app assistants such as Sabrina. These features are designed to organize, summarize, and educate — not to practice medicine. AI transparency language in these Terms satisfies any prior standalone AI Transparency Policy acceptance for platform access.

What AI-assisted features do

Depending on the feature, AI-assisted capabilities may:

• Summarize information you have already provided or uploaded.
• Organize inputs (for example, intake artifacts) to help you review and confirm.
• Identify patterns across your self-reported context to support educational framing.
• Generate non-binding educational insights and explanations tailored to the information available in the product.
• Personalize presentation order and surface-level guidance where product settings allow.

What AI-assisted features do not do

AI-assisted capabilities do not:

• Diagnose disease or conditions.
• Replace physicians or other licensed professionals.
• Guarantee correctness, completeness, or fitness for a particular purpose.
• Make autonomous clinical decisions for you.
• Verify that all uploaded data (images, PDFs, manual entries) is accurate or authentic.

Accuracy and limitations

AI outputs may be wrong, outdated, or misleading, including in ways that sound confident. You should treat outputs as starting points for learning, not conclusions.

Your responsibility for AI outputs

You remain responsible for decisions about medications, supplements, therapies, lifestyle changes, and any health actions. Always consult qualified licensed professionals for medical decisions.

AI processing and privacy

AI processing is subject to our Privacy Policy. Where optional features involve additional processing, the product will request consent where required.

9. No guarantees

Re:Formd does not guarantee:

• disease prevention
• performance results

Health outcomes depend on many variables beyond Re:Formd.

10. Use at your own risk

Use of Re:Formd and its content is entirely at the user's own risk.

Users assume full responsibility for how they interpret and apply information provided through Re:Formd.

Use of Re:Formd does not create a doctor-patient, therapist-patient, or healthcare provider relationship.

Re:Formd functions only as an educational and organizational tool.
